What is Advanced Life Support?

Advanced Life Support (ALS) is the highest level of pre-hospital emergency medical care in South Africa. Medi Edge ALS units are staffed by Advanced Care Paramedics — clinicians registered with the Health Professions Council of South Africa (HPCSA) and qualified to perform complex medical interventions independently in the field.

Unlike Basic Life Support, which focuses on stabilisation and transport, ALS allows our practitioners to administer a full pharmacological formulary, perform advanced airway management, interpret 12-lead ECGs, and initiate critical care protocols — all without needing a physician on scene.

Our units are strategically deployed across the greater Pretoria region, ensuring rapid response times and clinical capability exactly where you need it.

Gauteng DOH Licensed

Medi Edge is fully licensed by the Gauteng Department of Health to operate ALS emergency vehicles.

What Our ALS Units Can Do

Each capability below represents an intervention that can be performed on-scene or en route — without waiting to reach a hospital.

Cardiac Monitoring & 12-Lead ECG

Continuous cardiac monitoring with 12-lead ECG acquisition and interpretation, enabling early identification of STEMI, arrhythmias, and electrolyte disturbances.

Defibrillation & Cardioversion

Manual defibrillation for ventricular fibrillation and pulseless VT, plus synchronised cardioversion for haemodynamically unstable tachyarrhythmias.

Advanced Airway Management

Rapid Sequence Intubation (RSI), surgical airway access (cricothyrotomy), supraglottic devices, and end-tidal CO₂ monitoring for ventilated patients.

Pharmacological Formulary

Full pre-hospital drug formulary including vasopressors, antiarrhythmics, opioid analgesics, sedatives, bronchodilators, thrombolytics, and reversal agents.

IV / IO Vascular Access

Peripheral IV, central access facilitation, and intraosseous (IO) access in time-critical situations where IV access is not achievable.

Point-of-Care Diagnostics

Bedside blood glucose, SpO₂, ETCO₂, and 12-lead ECG interpretation to guide clinical decision-making in the field.

Indications for ALS

Conditions We Manage

ALS is appropriate whenever a patient's condition may deteriorate rapidly, requires pharmacological intervention, or involves complex airway or haemodynamic compromise.

Cardiac arrest (VF, pulseless VT, asystole, PEA)
ST-elevation myocardial infarction (STEMI) and ACS
Acute respiratory failure and severe asthma
Major trauma with haemodynamic instability
Stroke with altered level of consciousness
Anaphylaxis and severe allergic reactions
Diabetic emergencies (DKA, hyperosmolar states, severe hypoglycaemia)
Toxicological emergencies and overdose
Obstetric emergencies and eclampsia
Paediatric critical emergencies

ALS Unit Equipment

Every Medi Edge ALS vehicle is equipped and maintained to the full Gauteng DOH specification for Critical Care transport.

Monitoring

Cardiac monitor/defibrillator, 12-lead ECG, SpO₂, ETCO₂, non-invasive BP, temperature

Airway

Full RSI drug kit, IGEL supraglottic airways, BVM, CPAP, ventilator

Vascular Access

IV cannulas (all sizes), IO drill, infusion pumps, syringe drivers, fluid warmers

Pharmacology

Controlled and non-controlled drug safe: analgesics, sedatives, vasopressors, antiarrhythmics, reversal agents, bronchodilators

Immobilisation

Kendrick Extrication Device, vacuum mattress, long spine board, traction splint, pelvic binder
